Our initial evaluation is 60-90 minutes long and will consist of a conversation to discuss your concerns and goals. Tests and measures to find areas which need to be addressed, treatment for main concerns, and development of a plan moving forward.

Your care will be based on your needs and the treatment interventions we decide are a best fit for you. As opposed to insurance companies telling us what we can and can’t do. Your session will not feel rushed, and your care will be more individualized and detailed as your provider will have the time to treat you properly.

Your sessions are always one-on-one with the same provider. Sessions come to you at your home or preferred location adding convenience to your routine. Highly personalized care plans and focus on long term change sets this practice apart from others.

It can be less expensive over time as you may not need to be seen as often or for as long.

No! Indiana is a direct access state allowing physical therapists to be the first line of treatment.

The initial evaluation is $250 for 60-90 minutes.

The 1 hour follow up session is $225.

Package options are available for follow up sessions. When buying a package there is a discount associated.

Some people have out of network coverage and can be reimbursed through their insurance company. If this is a benefit you have and you would like to use, I can provide you with a superbill which you can then take back to your insurance company for reimbursement.

Mosaic doesn’t accept insurance. This is a private pay, fee for service practice. This prevents the typical restrictions insurances place on care. It allows for longer sessions and more individualized care focused on your goals.
