  • A 1-hour evaluation consisting of a discussion about your concerns and goals. Followed by testing to determine an individual plan of care consisting of varying forms of treatment.

  • Your care will be based on your needs and the treatment interventions we decide are a best fit for you. As opposed to insurance companies telling us what we can/can’t do. Your session will not feel rushed, and your care will be more individualized and detailed as your provider will have the time to treat you properly.

  • Your sessions are always 1:1 with the same provider. Sessions come to you adding convenience to your routine. Individualized focus on long term change and growth, where we value meeting your goals.

  • It can be less expensive over time as you may not need to be seen as often or for as long.

  • This will be variable depending on symptoms, starting point and your overall goals. We will come up with a plan together that fits you best.

  • A wide variety of conditions including but not limited to, back pain, headaches, pain in upper/lower extremities, sports injuries, instability, dizziness, and many more. If you are unsure, please reach out!

  • Dry needling is an intervention that involves the insertion of a small monofilament needle into the restricted tissue. It is a conservative way to manage symptoms as nothing is being injected.

  • Yes! PT sessions are eligible for payment through HSA/FSA accounts.

  • No! Indiana is a direct access state allowing physical therapists to be the first line of treatment.

  • The initial evaluation is $175 for an hour-long session.

    The 1 hour follow up session is $175.

    The 45 minutes follow up session is $135.

  • You can use the “Contact Us” page and fill out the necessary information. I will then reach out to you to find the perfect time to meet.

  • Cash, credit, debit, HSA/FSA and Zelle.

  • Mosaic doesn’t accept insurance. This is a cash-based fee for service practice. This prevents insurance restrictions and allows for longer sessions and 1:1 care.
